This is actually from Washington State Dept of Health. A Customer Services Specialist from the Dept of Health replied to the query I sent to the State website. Their Behavioral Risk Factor Surveillance System conducts an annual survey of residents health. You can reach them at 866-871-5405 regarding this survey.

From what I've read in the newspaper, this is a legitimate call about health from the State of Washington. I've never received a message. I don't answer any phone call if Caller ID doesn't provide a name. I think the State is highly unprofessional in not supplying an ID.
